Job Description

Under the direction of the Program Manager, the position of Counselor does the following:

  • Clinical Management.
  • Screening and Intake.
    • Assists Intake Coordinator with screens for new clients to determine client appropriateness for admission to the program and conducts intake interviews of new clients admitted to the program.
    • New Client Orientation.
    • Assists Intake Coordinator with orientation sessions with clients newly admitted to the program to ensure that all clients understand and acknowledge all program rules, regulations, and expectations.
  • Assessment:
    • Conducts diagnostic assessment interviews with new clients and prepares psychosocial histories.
  • Identifies client presenting problems and records diagnostic impressions in preparation for treatment plan development.
  • Treatment Planning and Implementation:
    • Develops initial and individualized treatment plans which address client presenting problems.
    • Develops targeted behavioral goals and appropriate treatment modalities.
    • Specifies target dates for attaining stated treatment objectives.
    • Monitors, evaluates, and documents client progress toward attaining treatment goals, and revises treatment plans as needed to enhance client outcomes.
    • Maintains case documentation in confidential files.
    • Follows standardized operating procedures and practices for service delivery that ensure client confidentiality, timely and appropriate care, and quality client outcomes.
  • Counseling Services:
    • Facilitates group therapy and provides individual counseling with fidelity to evidence-based practices.
  • Provides crisis intervention services to clients:
    • Functions as part of a treatment team to ensure that client needs are being systematically and comprehensively addressed.
  • Discharge Planning:
    • Prepares client discharge summaries and aftercare plans.
    • Makes referrals to other organizations or providers at the time of case closing, if appropriate during active treatment and/or at the time of case closings.
  • Case management:
    • Assists case managers in scheduling events, activities, and appointments for clients when needed.


Quality Assurance and Improvement:

  • Maintains complete, legible, and timely documentation of client progress.
  • Participates in Continuous Quality Improvement activities as stipulated in the agency's Continuous Quality Improvement Plan.
  • Meets deadlines for submission of timely, comprehensive, and accurate records for billing and record-keeping purposes.


Other:

  • Maintains productivity standards as defined by the agency.
  • Attends mandatory meetings and Completes all training requirements.
  • All other duties as assigned.


Credentials/Academic Requirements:

  • Proof of a Bachelor's degree in a human services-related discipline or comparable job experience must be provided.
  • Minimum of two (2) years of work experience in counseling or social work settings. 
  • LSW or LPC with Chemical Dependency focus.
  • Salary based on credential, degree, and experience
  • The candidate must be able to pass a BCII/FBI background check, drug screen, and obtain a Medicaid billing number.
